  • Medical Forms Pt. 1: Health Inventory

    Part I of the Medical Forms consists of three pages:

    Page 1 is completed by a parent/guardian

    Pages 2-3 includes a Health Assessment and must be completed by your child’s pediatrician.

    Note: Parts II and III of the Medical Forms (Immunizations and Lead Test) must also be completed by the pediatrician, so be sure to include all three forms when bringing them to the doctor!
